
Adolescent and Young Adult Cancer Transitions Program

Adolescents and young adults (AYAs) who survived cancer in childhood require lifelong, specialized care tailored to their unique journey. Yet, moving from a children’s hospital to an adult health system can be overwhelming. Without structured support, many AYA survivors risk “falling through the cracks” and missing out on the comprehensive care they need to stay healthy.
The program’s mission is to ensure AYAs experience a smooth, supported transition into adult health care. Key priorities include
(1) Personalized guidance to help survivors navigate the adult health system and manage their ongoing cancer care
(2) Connection to subspecialties so each patient remains up to date with their individualized survivorship care plan
(3) Expanded access to reproductive health services, including fertility consultations for every young survivor entering adult care
(4) Continuous feedback and evaluation, working closely with AYAs to ensure the program meets their needs and achieves lasting impact.
This grant represents a vital step toward empowering young cancer survivors with the skills, resources, and care they need to thrive for years to come.
